Wegovy FAQ: What People Ask Before and After Starting

Wegovy has the widest label of any GLP-1 medicine sold in the United States, and in the space of about a year it also gained a pill, a higher dose and a new federal price program. Most of what ranks in search about it is at least a year out of date.

This page answers the questions people search most, with a source on each. It is a reference, not medical advice: it will not tell you whether Wegovy is right for you, which product to ask for, or what dose to take. Bring those to a healthcare provider.

What is Wegovy approved to do?

Wegovy is semaglutide, the same molecule as Ozempic, sold under a different brand with different approved uses and a higher dose range. Its label now covers four things [1][2]:

• Weight management in adults with a BMI of 30 or more, or 27 or more with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.
• Weight management in children 12 and older with obesity, based on the STEP TEENS trial, where BMI fell 16.1 percent versus a small rise on placebo [9].
• Cardiovascular risk reduction in adults with established heart disease plus obesity or overweight. In SELECT, 17,604 such adults had roughly a 20 percent lower risk of cardiovascular death, heart attack or stroke over about three and a half years [6].
• Noncirrhotic MASH with moderate to advanced liver fibrosis, granted accelerated approval in August 2025, the first GLP-1 approved for that condition [14].

That fourth one is why Wegovy is not simply “the weight loss shot” any more, and the second and third are why it can be covered by plans that exclude weight-loss drugs outright.

What forms does Wegovy come in now?

Three, as of September 2026:

The pill has a rule that matters more than any dose number. The label directs taking it on an empty stomach when you first wake, with no more than 4 ounces of plain water, swallowed whole, and waiting at least 30 minutes before any food, other drink or other medication [21]. Absorption is very limited to begin with, and taking it with breakfast is a common reason people conclude it does not work.

How much weight do people lose on Wegovy?

Two numbers, and you need both.

In the trials. STEP 1 found average weight loss of 14.9 percent over 68 weeks on 2.4 mg, versus 2.4 percent on placebo [4]. Half of participants lost 15 percent or more. STEP UP, the higher-dose trial, found 18.7 percent on 7.2 mg versus 15.6 percent on 2.4 mg over 72 weeks, with about a third of the 7.2 mg group losing 25 percent or more of their body weight [7]. Novo Nordisk’s press materials lead with 20.7 percent for the same trial; that is the same data measured as if every participant had stayed on treatment, so the two numbers are not in conflict and neither is wrong. The pill trial, OASIS 4, found 13.6 percent at 64 weeks counting everyone, or 16.6 percent counting only people who stayed on treatment [8].

In ordinary care. A Cleveland Clinic analysis of 7,881 patients found average weight loss of about 8.7 percent at one year [10]. The same analysis explains the gap: 3.6 percent for people who stopped within three months, 6.8 percent for people who stopped later, 11.9 percent for people who did not stop. More than 80 percent were on a low maintenance dose.

The practical reading is that the trial numbers describe what happens when you stay on it at a full dose, and the real-world number describes what happens on average, including everyone who lost coverage, could not tolerate it, or could not afford it.

Should I move up to Wegovy HD?

That is entirely a prescriber’s call, and the label requires tolerating 2.4 mg for at least four weeks first [1][12]. What the evidence says is that the extra weight loss is real but modest, roughly three percentage points in STEP UP, and the tolerability cost is not trivial: gastrointestinal side effects rose from 61.2 percent to 70.8 percent, and dysesthesia, an odd tingling or altered skin sensation, went from 6.0 percent to 22.9 percent [7]. The FDA has said it is looking further into that skin reaction [12].

What does Wegovy cost, and why did my insurance drop it?

The list price was $1,349.02 per package in 2026, whether that package was the injection pen, the HD pen or a 30-tablet bottle [15]. Almost nobody pays that. Novo Nordisk sells directly at lower self-pay prices, and TrumpRx.gov advertised the Wegovy pill from $149 a month and the pen from $199 [20]. Novo Nordisk announced on February 24, 2026 that the list price would drop to $675 a month effective January 1, 2027 [17].

The self-pay numbers, as verified on September 14, 2026 [16]:

With commercial insurance that covers it, the manufacturer savings offer can bring the cost to as little as $25 a month, capped at $100 of savings per month, and government beneficiaries are excluded from that offer [16].

Coverage itself is the bigger problem. Through 2026 a large number of employer plans narrowed or dropped weight-loss drug coverage, and people who had been stable on Wegovy for a year or more found their prior authorization refused at renewal. Some plans added conditions such as documented ongoing progress or enrollment in a weight-management program.

If that happens to you, the sequence that matters is: get the denial in writing with the stated reason, ask the prescriber to submit an appeal addressing that specific reason, and know that most commercial plans offer an external independent review once internal appeals are exhausted. If the cardiovascular or MASH indication applies to you, that is a different coverage pathway than weight management and worth raising explicitly.

For Medicare, the picture changed on July 1, 2026. The Medicare GLP-1 Bridge demonstration gives eligible Part D enrollees certain obesity medicines, including Wegovy pens and tablets, at a flat $50 monthly copay through December 31, 2027, with a prior authorization request from the prescriber required [18][19].

What side effects should I expect?

Mostly stomach: nausea, diarrhea, vomiting, constipation and abdominal pain, plus headache, fatigue, indigestion, burping and gas [1]. They concentrate around dose increases and ease for most people at a steady dose. Constipation is the one most likely to hang around.

The Wegovy label also carries warnings that Ozempic’s does not emphasize the same way, including an increase in resting heart rate, and low blood pressure or fainting, often connected to fluid loss from stomach side effects [1].

The serious list is: pancreatitis, gallbladder disease including gallstones, acute kidney injury from dehydration, serious allergic reactions, worsening of diabetic eye disease in people with diabetes, and severe gastrointestinal reactions [1]. Every semaglutide product carries a boxed warning about thyroid C-cell tumors seen in rodents, and an absolute contraindication for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome [1].

On hair and muscle, the two questions people ask most: hair loss appears in the adverse reaction tables and is best explained as temporary shedding triggered by rapid weight loss rather than a direct drug effect, and lean mass does fall in absolute terms during weight loss even though its share of body weight rises [22]. Protein intake and resistance training are the levers people and their providers usually discuss.

What happens when you stop?

The STEP 1 extension is the clearest evidence. A year after stopping, participants had regained about two-thirds of the weight they lost, and blood pressure, cholesterol and blood sugar had drifted back toward where they started [5].

This is the single most important thing to understand before starting: Wegovy works while you take it. That does not make it a failure. It makes it a long-term treatment, and it makes the coverage question above a medical question rather than only a financial one.

Wegovy or Zepbound?

Zepbound is tirzepatide, a different molecule made by Eli Lilly, not a form of semaglutide. In SURMOUNT-5, the one large head-to-head trial, tirzepatide produced 20.2 percent average weight loss versus 13.7 percent for semaglutide over 72 weeks [11]. The trial was open-label, which is a recognized limitation.

Two things the efficacy number does not settle. Zepbound carries an obstructive sleep apnea indication that Wegovy does not. Wegovy carries cardiovascular and MASH indications that Zepbound does not. And for most people the deciding factor turns out to be which one their plan covers this year. That is a conversation for a prescriber, not a comparison table.
