At the Pharmacy Counter: Savings Cards, NDCs and Mail-Order for GLP-1s

A savings card is a second insurance claim, not a coupon. Here is the processing code your pharmacist actually needs, how to read the NDC to confirm you got the right dose and device, and how the two manufacturer mail-order channels work.

Most of what goes wrong with a GLP-1 prescription does not go wrong in the exam room. It goes wrong at the counter, or in a shipping box, and usually because of a mechanical detail nobody explained.

This article covers three of them: how a manufacturer savings card is actually processed, how to read the NDC on your carton to confirm you got what was prescribed, and how the two manufacturer-run mail-order channels work.

It deliberately does not quote current prices. Those change several times a year and are tracked in our access and pricing coverage, where they carry dates. What is stable enough to be useful here is the plumbing.

Why does the pharmacy say my savings card “doesn’t work”?

Because a manufacturer savings card is not a coupon. It is a second insurance claim, and it has to be submitted like one.

Novo Nordisk publishes the pharmacist-facing instructions on the back of the Wegovy savings offer, and they describe three different paths depending on your coverage situation [1][2]. SS&C Health is the processor, and the card carries its own BIN, PCN, group and member ID.

If you have commercial insurance that covers the drug:

“Submit the claim to the patient’s primary insurance first, then submit the remaining out-of-pocket balance to SS&C Health as the secondary payer using the BIN/PCN/GRP/ID found on the savings offer and with an other coverage code 08.” [1]

If you have commercial insurance that does not cover the drug:

“If Wegovy is not covered by the patient’s insurance, leave the primary claim as rejected and submit to SS&C Health as secondary payer using the BIN/PCN/GRP/ID found on the savings offer and with an other coverage code 03.” [1]

If you are self-paying, or processing outside your plan:

“Submit to SS&C Health as the primary claim using the BIN/PCN/GRP/ID found on the savings offer and with an other coverage code 00 or 01.” [1]

In every case, the pharmacy collects the reduced out-of-pocket amount shown on the adjudicated claim, and is reimbursed by SS&C Health.

Two more details from the same document [1]:

  • “Pharmacy must submit claim within 180 days from the date the prescription was filled.”
  • There is a Pharmacy Help Desk for processing questions: 1-844-373-0987.

Knowing the phrase “other coverage code” is often the difference between a card that “doesn’t work” and a card that processes. It is a routine field in pharmacy claim software; the technician may simply not have been told which value the card needs for your situation.

What are BIN, PCN and group numbers?

They are routing information. A pharmacy claim system needs to know where to send a claim, and those three fields tell it.

  • BIN (Bank Identification Number) identifies the claim processor.
  • PCN (Processor Control Number) identifies the specific program within that processor.
  • Group and member ID identify you within the program.

A manufacturer savings card prints all four, because from the pharmacy system’s point of view the card is just another payer. That is why the sentence “please run this as a secondary claim using the numbers on this card” is a more useful thing to say than “can you apply this coupon.”

Who is actually eligible?

Commercial insurance only, and this is the single most common reason a card is genuinely rejected rather than mis-submitted.

Every major GLP-1 savings offer excludes anyone enrolled in a state, federal or government-funded healthcare program. That includes Medicare (including Part D), Medicaid, TRICARE and VA benefits. Availability in Massachusetts is also dependent on state law.

If you are in one of those programs, the savings card is not the route, and no amount of resubmitting will change that. Coverage paths for Medicare and Medicaid patients are covered in our access and pricing material.

How do you read the NDC on the carton?

The National Drug Code is the most under-used verification tool a patient has. It takes ten seconds and it confirms you were dispensed exactly what was prescribed.

FDA explains the structure: the NDC is a three-segment number [6].

  • Segment 1, the labeler code, is “assigned by the FDA” and identifies the firm that manufactures or distributes the drug. Novo Nordisk is 0169. Eli Lilly is 0002.
  • Segment 2, the product code, “identifies a specific strength, dosage form, and formulation for a particular firm” and is “assigned by the firm.”
  • Segment 3, the package code, “identifies package sizes and types” and is also assigned by the firm.

FDA-assigned NDCs come in 4-4-2, 5-3-2 or 5-4-1 digit configurations. Billing systems pad one of the segments with a leading zero to reach eleven digits, because, as FDA puts it: “Because of a conflict with the HIPAA standard of an 11 digit NDC, many programs will pad the product code or package code segments of the NDC with a leading zero” [6]. That is why the number on your insurance paperwork may have one more digit than the number on the box.

FDA has also adopted a final rule moving to a uniform 12-digit format, effective March 7, 2033 [7].

Why it is worth checking. Because the product code carries the strength and the package code carries the presentation, every dose and every device has its own NDC. That makes the number a complete description of what is in the box [8][9]:

Look at the last four rows. Same drug, same 10 mg weekly dose, four different NDCs, four completely different devices with four different sets of instructions and three different storage windows. If your device changed this month, the NDC changed, and the label says a change of presentation should trigger new training and a reading of the new Instructions for Use [8][9].

The habit worth building: when you pick up a refill, check that the NDC on the carton matches the NDC on the pharmacy label, and that both match what you had last time. If they do not, ask before you open it.

One caution: an NDC tells you what the labeler says the package is. It is not by itself proof of authenticity. Counterfeit detection is covered separately in our compounding and safety material.

How does NovoCare Pharmacy work?

NovoCare Pharmacy is Novo Nordisk’s direct-to-patient channel for Wegovy, Ozempic and the oral semaglutide tablets. The flow it publishes has three steps [3]:

  1. Get a prescription. Your prescriber sends it directly to NovoCare Pharmacy, or you connect with a provider through the site. NovoCare’s instruction: “Have your prescriber send your prescription to NovoCare Pharmacy and look for your confirmation text.”
  2. Choose how you are paying. Self-pay, or through your insurance with the savings offer applied.
  3. Choose how you get it. Free home delivery, or pharmacy pickup.

CoAssist Pharmacy handles the dispensing. Both the injection pens and the tablets go through the channel.

What NovoCare’s public pages did not state when we checked on September 14, 2026: shipping speed, whether shipments are cold-packed, or whether pen needles are included in a pen shipment. Those are reasonable questions to ask when you enroll rather than to assume, particularly the cold-chain one for a refrigerated product.

How does LillyDirect work for Zepbound?

Differently, and the difference is that the device you want may determine the channel you have to use.

Lilly’s medical information page states that Zepbound vials and multi-dose KwikPens “are available in all strengths exclusively through LillyDirect’s self-pay pharmacy channel for patients with a prescription from their healthcare provider” [5]. The single-dose pen can also run through the LillyDirect insurance pharmacy component.

What LillyDirect publishes about the process [4]:

  • A valid, on-label electronic prescription from the provider of your choice is required.
  • Each monthly single-dose vial prescription contains four once-weekly vials.
  • “Needle and syringe purchase required” appears beside every vial strength. Injection supplies can be added to your cart at checkout or purchased separately.
  • You choose free home delivery or free retail pickup at Walmart Pharmacy.
  • Self-pay pricing is conditioned on refilling within 45 days.

Two things worth registering there. First, the syringes are a separate line item, so a price quoted for the vial is not the full cost of a month. Second, the 45-day refill condition means a lapse can change what you pay, which matters if you are traveling or between appointments.

What should I check before I walk away from the counter?

A short list, built from the label requirements and the processing rules above:

  1. Is this the same device as last time? Compare the NDC on the carton to your last box. Wegovy has four injection presentations and Zepbound has four; a switch is easy to miss and changes both the procedure and the storage window [8][9].
  2. If it is a different device, did anyone offer training? The labels require it. “If the prescribed presentation changes, ensure patients and caregivers receive appropriate training and instruct them to consult the Instructions for Use for the newly prescribed presentation” [8][9].
  3. Does the Instructions for Use in the box match the device in the box? Each presentation has its own.
  4. Do I have the supplies this device needs? Novo dial pens ship with NovoFine Plus 32G 4 mm needles. Lilly KwikPens do not include pen needles. Lilly vials do not include a syringe or needle at all [8][9].
  5. Do I have somewhere to put the used sharps? If not, Novo Nordisk sends a free container to patients on its products.
  6. If the card did not process, what code did they try? 08, 03, or 00/01 depending on your coverage, and there is a Help Desk number on the card [1].

The short version

  • A savings card is a secondary insurance claim, not a coupon, and the field that usually needs fixing is the other coverage code — 08, 03, or 00/01.
  • Cards are commercial insurance only. Medicare, Medicaid, TRICARE and VA are excluded, and no resubmission changes that.
  • The NDC is a labeler-product-package code, and it uniquely identifies the dose and the device. Check it every refill.
  • Zepbound vials and KwikPens come only through LillyDirect self-pay, and the syringes are a separate purchase.
  • NovoCare Pharmacy takes an e-prescription directly from your prescriber and offers home delivery or pickup.

None of this is about the drug. All of it is about getting the right box, at the right price, with the things you need to use it in it.
