Morgan Stanley downgrades Novo Nordisk over growth and the patent cliff

Morgan Stanley downgraded Novo Nordisk over its heavy reliance on semaglutide and a coming patent cliff, a shift that could affect drug pricing, availability and competition for Ozempic and Wegovy users in the next decade.

Morgan Stanley cut its rating on Novo Nordisk to Underweight from Equal-weight on September 11, 2026, warning that the company's growth outlook and its dependence on semaglutide, the active ingredient in Ozempic and Wegovy, look riskier than the stock price suggests [1]. The bank kept its price target at 250 Danish kroner, implying more than 10% downside from the stock's last close, and shares fell 2.6% in early Copenhagen trading [1].

The core concern is concentration. Semaglutide is expected to make up about 75% of Novo Nordisk's total sales in 2026 [1][2][3]. The drug's patent protection is set to lapse in Europe in 2031 and in the United States in 2032, opening the door to generic competition [3]. Morgan Stanley still models semaglutide supplying 59% of sales in 2031, meaning the exposure does not disappear right away, but the bank expects pricing and competitive pressure to build well before then [1].

Novo Nordisk's own guidance points to a tougher near-term picture too. The company has told investors to expect flat to 6% lower sales and operating profit this year at constant exchange rates [2][3]. In the most recent quarter, gross margin slipped to 78.2% from 82.7% a year earlier, and sales of the Wegovy injection fell 22% [3]. The company has said it serves roughly 46 million patients [3].

Morgan Stanley does credit Novo's newer oral obesity pill, but says it won't be enough on its own. The bank forecasts the oral franchise reaching $10 billion in sales by 2031, calling that "not enough to offset pricing and competitive headwinds" [1]. A survey of 200 primary care physicians commissioned by the bank pointed to growing GLP-1 use overall, but also market share losses for Novo over the next 18 months to Eli Lilly's current drugs and its upcoming retatrutide, expected in 2027 [1]. Novo has also had a pipeline setback: two Phase III trials of the heart-failure drug ziltivekimab were halted after an independent monitoring committee judged their prospects poor, though a third trial continues with data expected in the first half of 2027 [2].

Why it matters for patients

This downgrade is about investor returns, not drug safety or effectiveness, and it does not change what semaglutide does in the body. But the underlying facts touch patients in a few ways. A 2031-2032 loss of patent exclusivity in the U.S. and Europe means that, at some point in the next decade, generic versions of semaglutide could become available, which historically brings sharply lower prices for a drug that today is used by tens of millions of people [3]. In the meantime, Novo Nordisk's financial pressure could shape how it prices and markets Ozempic, Wegovy and its oral pill, and how aggressively it competes against Eli Lilly's Mounjaro, Zepbound and future drugs like retatrutide [1][3]. Slowing growth for the branded injection, reflected in the 22% sales drop noted above, may also be a sign of broader market shifts in how patients and insurers are choosing between GLP-1 options [3]. None of this is medical guidance, and patients weighing these medications should not read a stock downgrade as a comment on whether a drug works or is safe.

What happens next

Novo Nordisk plans to hold an internal town hall for employees on September 15, 2026, followed by a capital markets day in London on September 21, 2026, where management is expected to discuss its post-semaglutide strategy, pipeline priorities including MASH and chronic kidney disease, and its response to the looming patent cliff [1][3]. The company's third-quarter results are due November 4, 2026 [3].
