Wegovy pill becomes the most-chosen new weight-management start

Novo Nordisk says Wegovy, boosted by its new pill and a high-dose injection, became 2026's top choice for patients starting a new obesity drug, even as U.S. sales keep falling on lower prices.

Novo Nordisk says that over the course of 2026, more patients starting a new anti-obesity medicine chose Wegovy than any other option, a shift the company credits to its once-daily pill and a stronger injectable dose [1]. The company reported the trend at the American Diabetes Association's 2026 Scientific Sessions in New Orleans, alongside a report that oral Wegovy has now crossed 3 million U.S. prescriptions since its January 5 launch [1].

The pill's rollout has been fast. It reached 1 million prescriptions in its first 12 weeks, then added another 2 million in just 10 more weeks, working out to roughly one prescription filled every five seconds [1]. Novo calls it "one of the strongest US pharmaceutical launches by volume on record" [1]. Earlier company data put total prescriptions for the pill at around 1.3 million for the first quarter of 2026 and more than 2 million since launch as of mid-April, with weekly prescriptions topping 200,000 by the week ending April 17 [2].

A key figure behind the "most-chosen new start" claim: more than 80% of new Wegovy pill prescriptions are going to patients who had never tried a GLP-1 medicine before, according to Novo [1]. That matters because the company had hoped an oral option would draw in new patients rather than simply pull people away from its own injectable Wegovy [1]. Novo also says it saw "strong uptake" of Wegovy HD, the higher-dose 7.2 mg injectable version approved by the FDA in March and launched in April, which produced 20.7% mean weight loss in the STEP UP trial [1][2].

The pill itself, taken once daily at a 25 mg dose, produced about 17% mean weight loss in the OASIS 4 trial when patients stuck with treatment, with roughly one in three participants losing 20% or more [3]. That is similar to the weight loss seen with injectable Wegovy at its 2.4 mg dose [3]. The pill is also approved to reduce the risk of heart attack, stroke, and other major cardiovascular events in adults with existing heart disease and obesity or overweight, mirroring an indication already held by the injectable form [1][3].

The financial picture is more mixed than the prescription numbers suggest. Novo's pill brought in $355 million in its first quarter on the U.S. market [1], but overall U.S. sales fell 11% at constant exchange rates in the first quarter, driven by lower realized prices even as volumes grew [2]. Novo has revised its full-year 2026 sales guidance to a decline of 4% to 12%, a slightly smaller drop than the 5% to 13% decline it had forecast earlier in the year [1][2]. Some analysts, including teams at Jefferies and Citi, have warned that prescription tracking data may undercount telehealth referrals, which could affect how these launch comparisons are read [1].

Why it matters for patients

For patients weighing options, the pill's rapid uptake signals that many people now starting weight-loss treatment are choosing a tablet over an injection, which could ease a common barrier for people uncomfortable with needles. Novo executive Ed Cinca noted that an estimated 120 million people in the U.S. could qualify for an anti-obesity medicine, but only a small share currently receive one, pointing to cost, coverage, and stigma as ongoing obstacles [1]. Pricing details reported earlier this year put the pill's starting 1.5 mg dose at $149 a month for cash-paying patients, with commercially insured patients potentially paying as little as $25 a month through savings programs [1]. Novo has also cut self-pay prices for Wegovy and Ozempic and listed its medicines on the federal TrumpRx purchasing portal following a pricing agreement with the White House [1].

What happens next

Novo launched the Wegovy pill in the United Arab Emirates on June 3, 2026, the first market outside the U.S. to get it, and has said it plans to bring the pill to additional countries in the second half of 2026, guided by local demand and health infrastructure [3]. Regulatory decisions on the pill in Europe and other markets remain pending [2][3].
