Novo Nordisk confirms $675 US list price for Wegovy, Ozempic and Rybelsus in 2027

Novo Nordisk has confirmed that US list prices for Wegovy, Ozempic and Rybelsus will fall to $675 per month for all doses on January 1, 2027 ^[1]. The company describes the change as roughly a 50% reduction from the current Wegovy list price and about a 35% reduction for Ozempic ^[1].

The $675 figure was restated in the company's second-quarter 2026 earnings review, which says that from January 1, 2027 the US list price for Wegovy injection and tablets and for Ozempic will be $675, and that the cut will affect 2027 cash flow ^[2]. Novo Nordisk's announcement of the reduction names all three semaglutide medicines — Wegovy, Ozempic and Rybelsus — and says it applies to every available dose ^[1]. All three contain the same active ingredient, semaglutide ^[1]. List price is also called wholesale acquisition cost ^[1].

"Our actions today answer that call and remove cost barriers so the value of Wegovy and Ozempic can be realized by more patients," said Jamey Millar, executive vice president of US operations at Novo Nordisk ^[1]. "The lower list price is intended to connect more people with our innovative medicines, specifically those whose out-of-pocket costs are linked to list price, such as individuals with high-deductible health plans or coinsurance benefit designs."

Millar also said that lowering list prices is "the best approach" to reaching more than 100 million people living with obesity and more than 35 million people with Type 2 diabetes in the United States ^[1].

List price is a reference point used across the health care system, not the amount most patients pay at the pharmacy counter ^[1]. The company says the reduction does not affect its existing direct-to-patient self-pay pricing programs ^[1]. Self-pay has grown to about 35% of US injectable Wegovy prescriptions, up from 10% to 15% a year earlier, and self-pay sits at a lower price point than insured channels ^[2]. Novo's oral Wegovy pill is sold at $149 to $299 per month depending on dose ^[2].

The cut arrives as Novo Nordisk's US business is shrinking on price. US Wegovy injectable sales fell 22% at constant currency in the second quarter of 2026, even though weekly injectable prescriptions rose to about 310,000 from roughly 280,000 a year earlier ^[2]. The company's pricing is also shaped by a most-favored-nation agreement and by Medicaid coverage reductions in several states ^[2].

Why it matters for patients

Most people with insurance do not pay list price, so the direct effect of a lower list price depends on how a plan builds its cost sharing. Where a plan uses list price to set coinsurance or to track a deductible, a lower list price lowers the dollar figure those calculations start from — the benefit the company points to for people in high-deductible plans or coinsurance designs ^[1]. The company has not said what will happen to net prices after rebates, which are not public, or to copays, and whether insurers and pharmacy benefit managers pass the lower list price through has not been reported ^[2].

Cash-paying patients may see little from this announcement, because Novo Nordisk says the change leaves its self-pay programs alone ^[1]. A separate change could affect which products are covered: effective October 1, 2026, CVS Caremark will add other weight-loss medications to its largest commercial template formularies, ending an exclusive position Novo Nordisk held at a pharmacy benefit manager with roughly 26% of the US market ^[2].

Rybelsus is approved for Type 2 diabetes. Ozempic is approved for Type 2 diabetes, to reduce cardiovascular risk in certain patients, and for chronic kidney disease in adults with Type 2 diabetes. Wegovy is approved for chronic weight management and to reduce the risk of major cardiovascular events in adults with known heart disease and obesity or overweight ^[1].

What happens next

Two dates are set. On October 1, 2026, the CVS Caremark formulary change takes effect ^[2]. On January 1, 2027, the $675 list price takes effect for all doses of Wegovy, Ozempic and Rybelsus ^[1][2]. Medicare Part D's "Bridge" pilot, which began July 1, 2026 and covers a majority of beneficiaries at a $50 copay, is already running ^[2].

Not yet known: how much of the list price reduction reaches patients through lower copays, whether other GLP-1 manufacturers match it, and what the change does to Novo Nordisk's net revenue in 2027. The company has said only that it will affect 2027 cash flow ^[2].
