Lilly expands Zepbound KwikPen self-pay pricing to pharmacies nationwide

Eli Lilly announced on March 16, 2026 that self-pay pricing for the Zepbound (tirzepatide) KwikPen is now available at major pharmacies nationwide, as well as through the company's LillyDirect pharmacy, starting at $299 a month for the 2.5 mg dose ^[1][3]. Zepbound is a tirzepatide injection, not a semaglutide product.

Lilly also introduced a KwikPen Self-Pay Savings Card. The company says the card is available to all patients, including people without insurance, and that adults with a valid, on-label prescription for the KwikPen can use it to access the same self-pay prices offered through LillyDirect ^[1][3]. The card applies only to the KwikPen; it does not apply to Zepbound single-dose vials or the single-dose pen ^[1][3].

The card's terms describe it as a program for self-paying (cash) patients. Cardholders agree not to seek reimbursement from any third-party payer, including private insurance or state or federal health programs, and not to apply those costs toward a deductible. Users must be 18 or older and live in the United States or Puerto Rico ^[2]. The card can be used for up to 11 prescription fills per calendar year and expires December 31, 2026 ^[2].

Prices by dose, according to Lilly's provider savings page ^[2]:

• 2.5 mg: as low as $299 for a one-month fill.
• 5 mg: as low as $399.
• 7.5 mg, 10 mg, 12.5 mg, and 15 mg: as low as $449 under the Zepbound Self Pay Journey Program.

The 2.5 mg dose is a starting dose, not an approved maintenance dose ^[1][2]. A month is defined as 28 days and up to one KwikPen, which holds four doses ^[2].

The $449 price for the higher doses comes with conditions. Lilly says it applies automatically to a first purchase beginning February 23, 2026, but to keep it, a refill must be completed within 45 days of the delivery date of the previous fill. Otherwise regular prices apply, which Lilly lists as $499 for 7.5 mg and $699 for 10 mg, 12.5 mg, and 15 mg ^[2]. Taxes and fees may apply ^[2].

Lilly's terms state that savings may not be applied at the point of sale and may require further action to obtain them ^[2]. Maximum monthly savings range from $65 on the 7.5 mg dose to $271 on the 10 mg, 12.5 mg, and 15 mg doses, $215 on 2.5 mg and $115 on 5 mg ^[2].

Why it matters for patients

For adults who pay cash for obesity treatment, this adds a retail-pharmacy route to prices that were previously tied to LillyDirect. The restriction to the KwikPen is significant: people filling prescriptions for Zepbound single-dose vials or the single-dose pen are not covered by this card, and Lilly has not said whether a similar card will be offered for those formats ^[1][3]. Lilly refers to the program as not insurance, and it cannot be combined with insurance coverage or counted toward a deductible ^[2].

Cost comparisons depend on coverage. Lilly's provider page lists eligible commercially insured patients with coverage starting at $25 a month, and says eligible Medicare Part D beneficiaries with a Zepbound weight-management prescription may pay no more than $50 a month, with terms applying to both ^[2].

The list of participating pharmacies is described as "major pharmacies nationwide" ^[1][3]. GoodRx said on April 9, 2026 that it is offering Zepbound KwikPen self-pay pricing starting at $299 a month through more than 70,000 pharmacies nationwide ^[4].

What happens next

The KwikPen Self-Pay Savings Card expires December 31, 2026, and Lilly has not said whether it will be extended ^[2]. Lilly says commercial CVS Caremark coverage expands October 1, 2026, with some employers reinstating coverage earlier ^[2]. Whether the listed self-pay prices will change before the card expires has not been disclosed.
